Greater Manchester Eligibility

Check the eligibility criteria for one of the programmes below. If you would like to join our Tier 3 Weight Management programme then you will need to speak to your GP.
If you would like to sign up your child to join Junior PARS, you need to speak to a Health Professional, or you can use the referral form on our Greater Manchester page.
For our Maternal Healthy Lifestyle Service, check your eligibility below, if you want to join, go to the referral form, download it and send it to morelife.gm.sawm@nhs.net.
Tier 3 Weight Management
- Live in Manchester, Salford, Stockport, Rochdale, Tameside and Glossop, or registered with a local GP
- Have a BMI of 35kg/m² or above or...
- Have a BMI of 35kg/m² or above and co-morbidities, such as: Type 2 Diabetes, Metabolic Syndrome, Hypertension, Obstructive Sleep Apnoea, Infertility and Depression if specialist advice is needed regarding overall patient management
- Have a BMI of 50kg/m² who are being considered for bariatric surgery or...
- Our programmes are FREE for adults living in Bury or registered with a local GP and have a BMI of 40kg/m² or above and looking to have bariatric surgery
Junior PARS
- The young person must be a resident or have a GP practice within the Manchester postcode area
- Must be aged between 4-17 years old
- The young person must be above a healthy weight (>91st centile)
- Must not have a chronic condition that requires rehabilitation
- Must not currently be active to recommended levels

Maternal Healthy Lifestyles Programme
- Live in Salford or have a GP in Salford
- Be 6 weeks or more pregnant and registered with a midwifery provider
- Aged 18+
- BMI > 30kg/m2
- You can also speak with your local Midwifery team or GP practice for referral
