Tier 3 Weight Management
- Live in Medway or Swale
- Aged above the 98th BMI percentile
- Referrals to this service are predominantly made by the local NCMP, but can also come from your GP
Exclusion Criteria:
- Children who do not meet the 98th and above BMI percentile criteria
- Children with an acute medical condition and undergoing investigative work to establish a diagnosis and/or treatment plan
- Children and Families unwilling to commit to the intervention time period
Post-Partum Weight Management Service
- Aged 16 and over (<18 with parental consent and support)
- Registered with a Medway GP
- Given birth within the last 24 months
- BMI > 25kg/m2
