Tier 3 Weight
Management
Live in Sussex and aged 18+
Have a BMI of 40kg/m² or above
Have a BMI of 35kg/m² or above with co-morbidities
Have a of 30kg/m² or above, Type 2 Diabetes and are considering bariatric surgery
Have a of 27kg/m² of Black or Asian ethnicity and recent-onset Type 2 Diabetes Unsuccessful weight loss or maintenance at Tier 1 and 2
Pre-bariatric surgery (Tier 4)
Pre-knee or hip surgery for MSK patients
Have a BMI of 40kg/m² or above
Have a BMI of 35kg/m² or above with co-morbidities
Have a of 30kg/m² or above, Type 2 Diabetes and are considering bariatric surgery
Have a of 27kg/m² of Black or Asian ethnicity and recent-onset Type 2 Diabetes Unsuccessful weight loss or maintenance at Tier 1 and 2
Pre-bariatric surgery (Tier 4)
Pre-knee or hip surgery for MSK patients
